Dear Applicant,

Thank you for your interest in becoming a driver with circYoulation. We are excited about the possibility of working together. This letter is to confirm our intent to move forward with your application for an Independent Contractor Driver position, pending successful completion of the credentialing process.

As a circYoulation driver, you will serve as a 1099 independent contractor, providing non-emergency medical transportation for our riders across North Carolina, South Carolina, and Georgia.

Key Points of the Opportunity:

Pay Structure: $14 per hour from your first pick-up to your last drop-off.

Contractor Incentive Program (CIP): After 40 hours in a week, your rate increases to $21 per hour—similar to overtime.

Schedule: Operations run between 3:30 AM and 7:00 PM, Monday through Saturday. You will not drive the entire window but must be available during these hours as assigned.

Vehicle & Fuel: circYoulation provides the vehicle. Contractors contribute $10 per day for lease and $10 per day for fuel (with a company fuel card provided). Costs apply only on days you drive and are tax-deductible.

Requirements:

Valid driver’s license and clean driving record.

Ability to pass a drug screen (and DOT physical in South Carolina).

Completion of background and insurance checks.

Enrollment in a AAA Roadside Assistance plan.

Training: Includes online courses, in-person training, and final state broker approval before assignment.

This is not an offer of employment. circYoulation drivers are independent contractors responsible for their own taxes, benefits, and compliance with state requirements. This letter serves only as a statement of intent to proceed with your application and outline the terms of engagement if approved.

I understand that as part of credentialing, I must complete a background check, insurance check, and drug screen (plus a DOT physical if in South Carolina). As a 1099 contractor, I am responsible for covering these costs.

Commitment
​I understand that this work is a serious responsibility. Riders depend on me for safe, reliable transportation to medical appointments, and I am committed to fulfilling that responsibility.

Independent Contractor Status
I understand that circYoulation drivers are 1099 independent contractors, not employees, and I am responsible for my own taxes.

Vehicle & Fuel
I agree to pay $10 per day for vehicle lease and $10 per day for fuel. These costs only apply on days I drive and are tax-deductible.

Roadside Assistance
I agree to participate in the AAA Roadside Assistance plan to cover lockouts, flat tires, and dead batteries.

Training & Credentialing
I agree to complete all required online courses, in-person training, and credentialing steps before receiving state approval (“in the system”).

Final Acknowledgment

​By submitting this application, I confirm that I have read and understood all the requirements for becoming a circYoulation driver. I understand that this is an independent contractor role, and I am prepared to commit the time, effort, and responsibility required to serve riders who depend on me.
